How do spammers get past the confirmation code

In the last week or two I've seen a wave of new registered uses who are clearly bogus. A year or so ago I started using the 'confirmation code' which has worked great until recently.
How are spammers getting past the 'confirmation code' ? Are they doing it manually?
Any comments or hints would be appreciated.
I am using 2.0.19, no MODS.

I imagine some do it manually. Or, at least part of the process is manual. Try using a CAPTCHA feature if one is available to you.

I think they somehow knw working of the software and they know that which captcha it may produce because I have few forums.. everywhere they are passing away even from image-verification.. ultimately had to put account activation by admin.
